Prescribing

The Kent and Medway Area Prescribing Committee (K&M APC) first met in November 2002. Its membership comprises representatives from the three primary care trusts, four acute trusts and the partnership trust within the Kent and Medway Strategic Health Authority area, plus the Kent Local Medical Committee and Kent Local Pharmaceutical Committee.

MM1 Form - Standard

Use this form for initiation of treatment with high cost drugs, notification of withdrawal of treatment or transfer to the GP unless there is an MM1 variant specified for the drug/disease area listed below:
