Registering with a new doctor

To be able to make an appointment to see a doctor, patients generally need to be registered with that surgery first. Every citizen has the right to be registered with a local GP. Most people need a new GP because they have moved house. This is how you register with a new doctors' surgery

 

  1. Phone or visit the practice of your choice and ask to be registered.
  2. If the surgery agrees to add you to their list of patients, you will be asked to fill in an application form with details about yourself and your previous GP. It is helpful if you have your NHS medical card or your NHS number handy.
  3. You may be invited to attend a new patient medical assessment.
  4. You will receive a new medical card in due course.

 

If your chosen GP surgery cannot take you as a patient.
